•With the term Aging-in-Place, solutions are searchedto create opportunities for the elderly to live in theirhomes in familiar living environments.

•Thanks to the Aging-in Place solutions, the elderly donot have to move when their service needs change.

•Living at home is supported by different kinds of livingand care arrangements.

Services for elderly living at home

•The aim of elderly services isto ensure that clients over 65years of age can get alongand live at home.

•The social workers and socialinstructors from the localsocial and contact work unitsmap the service needs of theelderly as well as instruct,guide, support and settle thesocial problems of the clients.

Services for elderly living at home

•Social workers manage applications for apartmentrenovation (disability service handle theapplications of the seriously disabled) and startthe settling and assessment, when the elderlyperson cannot get along at home even withsupport from the help services.

•The social instructors from the social and contactwork units provide support and look for solutionsin different life situations together with the client.This includes house calls and direct groupactivities. For example, they help to makeapplications for benefits or memory examinations.

Services for elderly living at home

Every inhabitant of the municipalitywho is 75 years of age has a legalright to an assessment of their socialservice need. The assessment isperformed by a social worker during ahouse call, and when necessary, withthe assistance of the home careworker. Social workers and socialinstructors make annual preventivehouse calls to elderly people of 75years of age who do not regularly usethe social and health services.

Family caregiver support

•A person who cares for an elderlyperson at home can receive familycaregiver support, the amount ofwhich is determined based on howtime-consuming and demandingthe care is. The carer can be thenext of kin, a relative or someother close person. Familycaregiver support also includesservices to support the caregiver’sresolution.

Day activities

•The aim of the day activities is tosupport the customer's living athome and to facilitate the everydaylife of the family carer.

•Day activities for the elderly consistof rehabilitating care, intendedmainly for people in family careand people suffering from memoryproblems. Day activities includereturn trips between home and theday activity unit, meals, and theactivities. The groups are Finnishor Swedish.

Service and recreation centres

•The service and recreationcentres offer pensioners and theunemployed a chance to care fortheir wellbeing. There are 12centres, which offer differentkinds of activities and services,such as hobby and study circles,exercise, peer support groupsand guidance services.Additionally, all centres have acafeteria and meal services.

Housing for the elderly, service housesand old-age homes

•The purpose of service houses andold-age homes is to care for elderlypeople who require 24-hour care.

•The employees of the SocialServices Department’s social andcontact work unit, together withhealth centre personnel, guidesenior citizens into service housesand old-age homes, either for ashort-term period of treatment or forlong-term residence or care.

Rehabilitation and evaluationservices

•Multiple/various and preventativeservices

•Assessment and rehabilitativecare: Assessing client physical andmental health abilities andpotential resources. During theevaluation, the customer'sfunctional capacity andrehabilitation possibilities areanalysed and a personal careplanis prepared.
