The remaining study indicator measuredoutpatient follow-up with a mental healthpractitioner within seven days of discharge
In FY 10-11, the second remeasurement wasreported
Based on the results, it was determined that thePIP would continue

Interventions
The PMHPs implemented plan-specificinterventions
There was discussion of a possible collaborativeintervention in 2008 and the issue was  revisited in2010
In April 2011, a collaborative intervention wasagreed upon by the PMHP group
The collaborative intervention is follow-uptelephone contact with members after an inpatientdischarge

All except one of the PMHP PIPs reported thirdremeasurement rates that demonstratedimprovement from baseline
