1.Fill out the name of the event-RFL of Kershaw County, SC
2.Fill out the name of your team.
3.Fill out the name of your Team Captain.
4.Fill out the phone number of the Team Captain to contact if there are questions.
5.Fill out the Team captain’s e-mail.
6.Be sure to enter the Team member’s name in the first column if they are toreceive credit. (***They must be registered ONLINE in order to get credit socheck the site first). If they are not registered or no name is listed in the TeamMember Name column then the amount will be credited to the TEAM. We can’tgive credit to a team member that isn’t registered.
7.Denote ALL sponsors by filling them out as listed on this sample. (Rememberthat if a sponsor is listed on the accounting sheet then there should be asponsor form filled out as well.)
8.Put all luminaria money in the luminaria column. (Remember that if there isluminaria listed on the accounting sheet then there should be a luminaria formfilled out as well.)
9.Team fundraising amounts or donations go in the team member donationscolumn. These include bake sales, car washes, feet, cupcakes etc.)
10.Be sure to total all your columns.
11.All columns should add up to your total amount.
12.Your total amount should agree to the total money enclosed in your donationenvelope. (CASH+coins+checks)
13.Your accounting sheet TOTAL should agree to your donation envelope total.
14.Sign your accounting sheet at the Team captain Signature line.
15.Keep a copy of this for your records.
16.If there is a discrepancy, we will notify you so that you can correct your records
17.Please fill up the sheet. It’s not necessary to have separate sheets for each item.Let’s not waste the supplies. They cost money too.
