This clinic can be booked viareception and is an alternative toseeing the GP

(this phone will be answered between 10.30-12 and 2.30-4pm)

SATURDAY MORNINGCLINICS – CHANGES….

In response to the patient survey, theSaturday morning clinics will bereplaced by an extended hoursservice on Wednesday evenings until7.30pm.

WEDNESDAY EVENING CLINICS

Appointments will be available withthe Doctor, Nurse, Emergency CarePractitioner or Health Care Assistantuntil 7.30pm

SURGERY CLOSURE-STAFF TRAINING

The Practice will be closed for essentialstaff training on the following dates -

Thursday 17th September 1-6.30pm

Wednesday 14th October 1-6.00pm

(followed by late opening 6.00-7.30pm)

Thursday 12th November 1-6.30pm

Wednesday 9th December 1-6.00pm

(followed by late opening 6.00-7.30pm)

CONSENT

Informed implied or expressed consent will beobtained before any procedure or examinationis carried out.

An explanation will be given to the patientsufficient to understand the procedure.

Should the procedure carry a risk that thepatient is likely to consider as beingsubstantial, expressed consent (written orverbal) will be obtained. Copies of the consentpolicy are available on request.

ZERO TOLERANCE

The Practice operates a zero tolerance policy

We will not hesitate to remove patients from our listwho:

•are violent or abusive to any Practice member.

•use or condone threatening or violent behaviour.

•cause damage/steal from the Practice'spremises/staff/patients.

•obtain drugs and/or medical services fraudulently.

We ask you to treat your GPs and their staffcourteously at all times.
